Our Patient Referral Method is Simple and Quick
ONLINE REFERRALS:
PAPER REFERRALS:
Please download the referral form (PDF) for the Cavendish Imaging Centre you wish to use and return it to us by email, fax or post (contact details on the form itself).
REFERRALS ON HEADED PAPER:
We accept referral letters on headed paper that contain the following information:
- referrer contact details
- patient’s name
- patient’s date of birth
- the area of interest
- type and purpose of examination
- whether a radiology report is needed
Your question(s) to the radiologist must be as precise as possible. Please always give the patient’s medical history.
Your referral on headed paper can be emailed, posted or faxed to the relevant imaging centre.

All our centres are registered with most medical insurance companies: this makes it easy for your insured patient.
Cavendish Imaging Ltd is registered with the Care Quality Commission – no patient should be referred to a non-CQC-registered imaging centre.
